Constipation: Causes and Cures




Unfortunately, constipation is no laughing matter and it has caused me no end of worry. (Hence the lovely text messages to my hubby while he's at work.) For some reason, I gauge my success as a mom on the details of his BMs! But, now that Jett's 2 years old, I've finally figured it out! He goes every single day with no pain and perfect color and consistency. (I'll spare you the details!) So please allow me to share what I've found so you can get this problem solved more quickly... But, first:

WARNING: DO NOT USE MIRALAX. I realize that your doctor probably has recommend this for your child but, Miralax has never been approved for children and Miralax causes damage. Please read The Role of Miralax in Autism, Alzheimer's and Dementia. And Miralax's connection to “neuropsychiatric events.”

Safe Solution Summary
#1 treatment: Get your child's thyroid tested and properly treated. In the meantime...
For babies: consider trying Digestion Support Herbal Oil from Heritage Essential Herbs. It seems expensive, but not when you think about how long it lasts and, more importantly--it works! I simply rubbed a couple of drops on his stomach in a clockwise motion around his belly button at least three times a day (easy to do after diaper changes).  
Immediate Solution: buffered Vitamin C (to bowel tolerance) and/or Magnesium (also to bowel tolerance). Click the links to see how to use it and what the recommended brands are. Works great every time! Jett takes both everyday.
Long Term Solution: I also give Jett probiotics and enzymes as the long-term solution to constipation since it helps the gut to heal itself and support his body so that he will be able to properly metabolize and dispose of the food he eats. (If I haven't posted on enzymes yet, please email me and I will send you my post draft explaining it.) Treating the thyroid, however, is the number one long-term solution.
Constipation is such a common problem with our kids for many reasons. Two Moms, Ali and Jayme, share their research and strategies in order to help us all out.

Why do our children have trouble with constipation?

First, constipation is a red flag for hypothyroidism, Celiac Disease, Hirschsprung and other digestive disorders. Please read this post on the thyroid and DS. Make sure your child is properly screened to rule those out or to get them treated as soon as possible. Otherwise, (and less likely,) constipation could be from dehydration, low physical activity, or diet (avoiding grains and dairy usually helps). Many people have seen some success by following a healthy diet plan like www.gapsdiet.com or similar. Sometimes just making sure probiotics are part of your plan will fix it in the long run.


"The direct cause of constipation is not enough water in the stool. This occurs because of the diet not having enough water-retaining elements (fiber) or because the stool is kept in the rectum too long, allowing the colon to reabsorb more water than usual. In children with Down syndrome, two factors exist to make constipation more likely: low muscle tone and decreased motor activity. Both of these make the colon more likely to retain stool for longer periods, leading to loss of water from the stool." --from Dr. Leshin's page on constipation at http://www.ds-health.com/constip.htm

But I think that low serotonin could also be a factor... Serotonin is found throughout the body, especially in the gut. That's why people with DS who go on Prozac usually become regular. (But then the constipation eventually comes back... long story... but it's because the serotonin is recycled instead of regenerated on Prozac and eventually kind of runs out.) People with DS frequently show low serum tryptophan levels (a precursor to serotonin). And 100% of kids w/DS in a study (see below) had levels of serotonin that were only 40% of what is typical.

Serotonin can be increased with 5-Hydroxytryptophan, (5HTP) the precursor to serotonin. It's a good one to consider especially given its low rate of adverse effects compared with selective serotonin reuptake inhibitors (SSRIs), better known to us as antidepressants, such as Prozac. However, a 1971 study34 found that giving 5-HTP may cause spasms in infants. The dosage that led to these symptoms was 1.0 to 1.4 mg/kg in an infant aged 6 to 8 weeks. The patient did well once the dosage was adjusted. Tryptophan is also helpful, but takes about 3 months to fully restore levels, as well as B6 (not good to give large amounts of this to people with DS, but a little is okay) and niacin. I give Jett tryptophan and a little B6.

Also, if your child is constipated, the actual act may be very painful and unpleasant so he may end up holding it in when he does feel the urge. Of course, that only makes the problem worse.

Ali shares: 

We did go to a GI doc and he couldn't figure out a reason for our son's constipation. We found some supplements that work for him at the present. Others find success in higher doses of probiotics or with hefty doses of certain food items or with a combo of supplements, massage and pushing fluids. I think it can really vary from child to child.

Infant fleets or an oil enema

Once we realized it wasn't a good idea allow him to retain the poop, we made sure that Jack went daily -- with our help -- until we found our answer. [You don't want to wait longer than 3 days-Andi.] An olive oil enema to us was simply taking an empty bottle of this type http://www.walgreens.com/store/c/fleet-pedia-lax-for-ages-2-11-years-saline-laxative-enema/ID=prod3946475-product and filling it with olive oil... warming it a bit by setting it in hot water in a container... but not too warm... then in the most relaxed atmosphere we can create... pillows around, everything we need handy... maybe a favorite show or music on .... we'd use a bit of the oil as lubricant and then you insert gently and go slow keeping the bottle tipped so that you're not putting air in there. Once it's emptied as best you can you put on a diaper and wait. They can just play or whatever... we usually had success withing 10-15 mins.. Occasionally we had to do it again... but that was rare. The only thing we might change if we had to do an oil enema now is that I might try flax oil instead.... it's high in omega-3's and olive oil isn't.

What works with Ali's Jack


First I'll say what didn't work... massage, Fruit EZE, prunes, flax, cranial sacral, magnesium and a bunch of stuff I can't remember right now. Guess he's kind of an extreme case. :)

What does the job for him now.... a combo of Enzymedica DIGEST and also Renew Life CLEANSEMORE. The digestive aid helps break down the food and the Cleansemore contains rhubarb and triphala (combo of three Indian fruits) and it increases peristalisis (movement of stuff through the GI tract). If we give him only one it doesn't work. You need to research stuff.... know what your child is taking and its potential side effects. Don't go by just one website or study. There are some sources I trust a lot more than others. Also a good naturopathic doctor can be so helpful. Rhubarb is high in oxalates and may contribute to kidney stone formation. We work to counteract this possibility by giving Jack some Red Mineral Algae with each meal along with the Cleansemore and Digest. The Red Mineral Algae is a source of natural calcium. This can help prevent stones. :) Jack goes approx. twice daily on his own. I give him extra slippery elm with each meal (there's also some in the Cleansemore) because it's soothing to the digestive tract.

Fresh cabbage made Jack poop like crazy.... but strange behaviors began to kick in quickly with that so we had to stop it. (I've heard of this before in relation to cabbage & DS-Andi) Senna and Cascara Sagrada are too harsh and can cause harm when used long-term.

Mom, Jayme, to Vision (23 mos) shares her strategy

Find below some things that I have tried with moderate success and a couple of other things that have been really essential, in my opinion, in keeping Vision pooping every day.

Moderately successful
  • fresh peach sauce
  • not feeding anything made with any kind of flour, whole grain or not; so, no breads or pancakes or cakes or pasta
  • 2 or 3 TBSP of amaranth cooked with Vision's grains
  • prune sauce
  • not feeding bananas or apple sauce
  • lots of fiber
  • fresh vegetable juice made with carrots, parsley, bok choy, kale, chard, spinach, beets, (and sometimes pears or pineapple)

The clinchers, in my opinion

  • giving 2 Trienza (made by Houston Labs) enzymes with each meal (they are expensive, but worth every penny, IMHO; I buy mine at www.enzymeco.com)
  • giving 1/4 tsp sea buckthorn powder each morning (I got mine from www.znaturalfoods.com)
  • keeping him hydrated
  • giving him 1 tsp Inner Eco coconut water kefir daily
So, I am tossing out all this info. Look it over and do what you can do. If I could only do one thing, it would be the enzymes. They have made the most difference for him. They are also the most expensive thing on my list. The sea buckthorn powder would be the second thing on my list. He poops well every day, at least once, since I started him on it two months ago. It has also made a profound difference in his verbal processing and his physical development too. It has been a great supplement for Vision. Of course, it is hard to say if it would be as wonderful for your child, but think on all this.

...Before I knew about using essential oils, our baby daughter might go 7-8 days between movements if left to herself. I usually resorted to using glycerin suppositories to help her pass her bowels. Eat Your Way to a Healthy Colon
Health and disease both begin in the colon. If you struggle with constipation and want to heal your colon, start with what you eat.Eat foods that support colon cleansing:
* Avoid large amounts of salt, meat, and poultry. These are contracting foods that are more difficult to digest. We recommend that only 20-25% of a meal be animal protein. The rest of your plate should contain high-fiber vegetables, ocean vegetables and cultured vegetables.
* Eat lots of fresh vegetables. Lightly steamed, raw, baked...all veggies are an excellent source of fiber and several servings should be eaten at each meal The fiber in these foods helps push the food you've eaten through your intestines. 80% of each meal should be vegetables.
* Introduce sea vegetables into your diet. Rich in minerals they top the list with dark green leafy veggies for a daily must if you want to stay young and healthy. These often-overlooked vegetables provide trace minerals and nutrients that nourish both your thyroid and your adrenals. Weak thyroid and adrenals may be one of the most undetected causes of constipation and both organs need lots of minerals. A common cause of constipation is an under-active thyroid. Agar has been used in Japan to relieve constipation.
* Fruits. We do allow some limited fruit in the beginning stages of the Body Ecology Diet. As your candida symptoms disappear and you are certain your inner ecosystem is on its way to being established (by eating and drinking fermented foods and beverages), you can add others. These two are powders and can be taken by children to help them move their bowels.However, Donna feels it is important to have other types of magnesium supplements as well. Magnesium chloride from the Pain and Stress Center in Texas is one of our favorites. You can also purchase Magesium Asparate from your health food store in capsules. Take 400-1600 mg per day to help relax you and to help your bowels move on a regular basis.
* Eat garlic. Garlic is a healing, anti-microbial, cleansing food. Cook with it as much as possible and even swallow it whole.* Eat Body Ecology grain-like seeds.Cooked millet, quinoa, buckwheat and amaranth stimulate your colon, provide fiber and can ease constipation.
* Drink mineral water. If you feel too energetic at night to sleep and stay up late yet feel exhausted in the morning, you may be experiencing signs of dehydration.Yes, the inability to feel sleepy can come from not drinking water during the day. Solve the problem by drinking several glasses of mineral-rich water in those three to four hours between your dinner meal and bedtime. Drink two more glasses when you wake up. Keep track of your water intake during the day to make sure are well hydrated.Dehydration can be a sign of adrenal fatigue, however, and no amount of water will help until you heal your adrenals.

Probiotics: Benefits and Uses

Bifidobacterium lactis Dr. Usman pointed out in her AutismOne speech that it promotes neurogenesis--new cell growth in the brain. Supports digestion, helps prevent and stop diarrhea and improves immune function.

Lactobacillus acidophilus produces lactic acid (which inhibits yeast growth) as well as natural antibiotics, and has been shown to inhibit the growth of pathogenic bacteria such as salmonella, shigella, S. faecalis and E. coli. Studies have shown that L. acidophilus is effective in reducing lactose intolerance and reinforcing the immune system, as well as reducing cholesterol levels.

Lactobacillus brevis is a transient bacterium that is valued for its ability to help improve the symptoms of lactose intolerance. L. brevis inhibits the growth of some pathogens, and its proven antimicrobial effects support the immune system.

Lactobacillus bulgaricus is a transient strain that produces interferon, a protein that inhibits the replication of invading viruses, on its way through the intestinal tract. This bacterium is beneficial in cases of acute diarrhea and lactose sensitivity. Traditionally, L. bulgaricus is found in yogurt along with S. thermophilus.
Note: Has been proven to produce histamine, so avoid if needed.
 
Lactobacillus casei is commonly used for milk fermentation and is naturally occurring in the mouth and digestive tract. It breaks down carbohydrates and inhibits the growth of pathogenic bacteria in the small intestine. Studies have demonstrated L. casei's antimicrobial effects and its helpfulness in treating diarrhea caused by antibiotics and viruses. Note: Has been proven to produce histamine, so avoid if needed.

Lactobacillus plantarum secretes a naturally occurring antibiotic called lactolin, giving it beneficial antimicrobial activities. This probiotic is being studied for its efficacy against allergies.
It also breaks down histamine.
 
Lactobacillus rhamnosus occurs from the mouth to the small intestine, and helps inhibit the pathogenic bacteria involved in vaginal and urinary tract infections. It produces a mucus coating called mucin, which prevents toxins from entering the bloodstream and protects against the invasion of harmful microbes. L. rhamnosus has been shown to enhance the effectiveness of oral vaccinations against rotaviruses (which cause vomiting and diarrhea) and may help to remedy gastroenteritis (also called "stomach flu").
Shown to down regulate histamine receptors and up-regulate anti-inflammatory agents.

Lactobacillus salivarius found in the mouth and small intestine, has been shown to improve bleeding gums, tooth decay, bad breath, thrush and canker sores. L. salivarius breaks down proteins and produces B vitamins, enzymes and lactic acid. Its strong antimicrobial effects have been demonstrated clinically.

Bifidobacterium bifidum assists with proper digestion and absorption of calcium, iron, magnesium and other vitamins and minerals. It manufactures B vitamins and produces lactic and acetic acids, which inhibit the growth of unfriendly bacteria. Studies have shown that B. bifidum can reduce the incidence of acute diarrhea in infants and is beneficial in the treatment of inflammatory bowel disease. With regular use, B. bifidum can improve immunological and inflammatory responses in seniors.

Bifidobacterium breve also produces beneficial lactic and acetic acids. It has been shown to repress the growth of ulcer-inducing bacteria. Recent studies have demonstrated that B. breve populates the immature bowel of infants and can help very low birth weight babies to gain weight.

Bifidobacterium infantis is the most important probiotic found in infants' colons and is effective in the treatment of acute diarrhea. It stimulates the production of immuno-modulating agents, and can fight such pathogens as clostridia, salmonella, and shigella.
It also breaks down histamine.
 
Bifidobacterium longum plays a direct role in preventing invasion of pathogenic and putrefactive bacteria. B. longum breaks down bile salts, helps to synthesize B vitamins, and stimulates the immune system through its effects on immunoglobulin A (IgA). Another producer of the good acids (lactic and acetic), B. longum has been clinically shown to improve lactose digestion and constipation.
It also breaks down histamine.
 
Lactobacillus lactis (also referred to as Streptococcus lactis) is another transient bacterium that produces the immuno-supportive lactic acid. Lc. lactis produces a peptide called nisin, which has proven activity against C. difficile. In addition to controlling acute diarrhea, it is also recognized for its potential in the long-term management of colitis.


Streptococcus thermophilus is a transient bacterium with proven antioxidant activity. It produces large quantities of lactic acid and helps in the prevention and treatment of diarrhea. S. thermophilus also breaks down lactose and improves lactose intolerance by producing the enzyme lactase.
Shown to down regulate histamine receptors and up-regulate anti-inflammatory agents. Note: I avoid this strain. There is no substantial data to support my wanting to avoid it other than parents whose kids have PANDAS (which a lot more kids have it than we know about... they don't get diagnosed until their behavior is noticeably bad) say that their kids have a reaction to the strep strain in probiotics and yogurts. I'm not taking any chances. :)  

Lactobacillus gasseri is in some of Garden of Life's probiotics. These bacteria are thought to help the body function properly by reducing harmful bacteria, enhancing immune function, and stimulating digestion. It is often used to promote weight loss, reduce belly fat, inhibit fat production, speed up metabolism and protect against obesity.  It is said to treat allergies, asthma, diarrhea, high cholesterol, menstrual pain.

Saccharomyces boulardii, is an important yeast that functions like a probiotic in the body. Doctors have recommended it to help patients with diarrhea. It helps to regulate the intestines and protect them from pathogens and other things that can damage the intestinal lining. It also modulates different parts of your immune system and keeps the intestinal barrier function up and running. Studies show that taking S. boulardii alongside your antibiotics can reduce the possibility of getting diarrhea. S. boulardii can also help treat other ailments such as lactose intolerance, and might even have a future in the treatment of acne, vaginal yeast infections, high cholesterol, fever blisters and canker sores. It seems to improve the quality of life of people with irritable bowel syndrome (IBS), as well as Crohn's disease. According to Dr. Sid Baker, "For some individuals on the autism spectrum, the combination of S. b., a yeast-free diet, and avoidance of refined carbohydrates is all it takes to produce a dramatic improvement." Jett's Supplement List

Happy, healthy Jett at 3 years old
Below are the supplements that Jett currently takes (it does fluctuate) to combat the effects of the 21st chromosome. Please click the links for explanations as to why, how, our experience, products etc. 

You may notice that my list is different than what is suggested in the Changing Minds Foundation website (This protocol is outdated and I don't agree with the pharmaceuticals suggested), NuTriVene's Complete Program (minus the folic acid, Piracetam, Aricept and Namenda) or LifeExtension's Treatment of DS Summary (minus the glutamine, for most kids, but Jett actually takes it). This is because the more I learn, the more I am able to pick and choose what works best for Jett.





Common Questions 
 
Where do I start?

It may be a good idea to start with a Down syndrome specific multivitamin that has been formulated by experts. All of our children have the same diagnosis, but none of our children exhibit exactly the same symptoms. So it makes sense that while many of our kids may need many similar nutrients, which a multivitamin will cover, each child's perfect blend of nutrients will be unique to him/her. So, consider starting with a multivitamin and then add supplements as needed. Some of these multivitamin companies will customize the vitamin dependant on lab results of your child as well.

How do I know what my child needs?

Each of our children is unique, so your list will eventually evolve according to your child's cues as well. There are several tools for making these decisions:
1. Get Lab Tests

2. Check for Signs of nutritional deficiency

3. Get muscle testing and or tested from a zyto machine technician

4. Compare similar children's protocols at Autism360.org

5. Keep track of your child symptoms.
You can muscle test your child to see if he can tolerate supplements that you are interested in before he even tries it. Muscle testing is available at some chiropractors, naturopaths, NAET practitioners and integrated medicine practitioners. Here's a youtube video that explains how to muscle test. This technique has worked well for Jett. 

If your child muscle tests negative for an item that you feel would benefit him, you can use NAET (Nambudripad's Allergy Elimination Technique) to "clear" the "allergen" so that your child can tolerate it. NAET has worked well for Jett. Kristen Morrison of Naturally Better Kids swears by this technique for her child with Down syndrome.

Email me and I will send you a document that will help you keep track of dosage, side effects and other observations specific to your child. The more information you gather on your child, the better you can decide what is and isn't working.

How can I support my child's intake of nutrients?

You'll want to make sure that your child is best able to metabolize these nutrients. If your child has untreated Celiac disease (more common in DS than is realized, in my opinion) or simply a sensitivity to gluten, you may want to consider avoiding gluten (found in wheat products). Gluten can irritate the fine hairs in the digestive system so much so that the hairs can fall out and cause your child's food to not be properly metabolized. So your child may be eating great foods, but isn't getting the nutrients from it because it just slides quickly by, instead of being caught in the fine hairs and slowly guided through the digestive tract. I'd hate for you to be spending a lot of money on supplements when your child can't even use them properly. Jett is gluten free to aid in digestion.

Milk can cause block your child's ability to properly process folate (in addition to other negative effects like congestion). You may want to consider avoiding milk. Jett is dairy and casein free as well. See Cerebral folate deficiency in Down syndrome for details.



Jett's Supplement List


Consider introducing a new thing every 7-12 days or longer (I introduce only one thing a month now) and don't introduce anything else new (food/drink) during that time.
Note any behavior, sleep, bowel movement and skin changes. Again, contact me and I will email you a chart to make it easier for you to track your child's reaction. These simple rules will help you to best see how each item effects your child.

Medications: What to Avoid & Alternatives

I know we'd love for our kids to be completely drug, radiation and procedure free, but somethings we have to venture where we'd rather not. Here are some tips to minimize damage when taking various drugs and undergoing different procedures.

Antibiotics to Avoid

Our children with Down syndrome shouldn't take any medications containing sulfa such as Pediazole, Gantrisin, Septra or Bactrim. These drugs have the potential to suppress the thyroid gland, which is usually low functioning in the T21 population. Use only if absolutely needed. Sulfa drugs can further deplete zinc in zinc-depleted children (also common in our population). If sulfa drugs must be used, then an additional 10 mg a day of zinc should be supplemented while the child is taking the sulfa drugs.

Instead, request Augmentin, Ceftrim, Biaxin or Zithromax (Z-Pack).

Protection from Antibiotics

Antibiotics should be avoided as much as possible because in order to stop infection, it kills the good bacteria as well as the bad bacteria. To have a properly functioning digestive system, we need our good bacteria. If your child must take antibiotics, make sure you give him probiotics as well. Anesthetics to Avoid


Avoid using the anesthetic Atropine for any surgery, eye exams or dental surgery. Our kids are quite sensitive to atropine. It is used during anesthesia if the heart rate drops too low. But only a tiny dose is needed in DS.

Nitrous Oxide 
Nitrous oxide is to be avoided with someone who has the MTHFR gene (The MTHFR gene is probably a factor in your child having DS or autism). http://www.ncbi.nlm.nih.go​v/pubmed/18580170http://ispub.com/IJA/8/2/12867
Nitrous oxide depletes already depleted B12 (extremely common in DS) and, if your child is autistic (up to 33% of kids w/DS may also be autistic), it can cause severe regressions. There's info on anesthesia and ASD on the ARI web site.


Painkillers to Avoid

Acetaminophen should be avoided (Tylenol, panadol and paracetamol etc.). It is hard on the liver and depletes Glutathione levels. More explanation here:

Eye Drops to Avoid

Make sure when you take your child to the eye doctor that they don't use drops with atropine in them. Side effects such as pupils dilating for over two days as well as signs of unease including groaning and irritability.


Supplements to Avoid

You may want to avoid tulsi (holy basil), chlorella, spirulina and ashgawandha due to its SOD (super oxide dismutase) content, which is usually an issue with our children.
Aloe- In a study assessing herbal effects on the thyroid, ingestion of 125mg/kg Aloe vera for 15 days in rats was associated with a decrease in serum T4 (-12.88%) and T3 (-25.13%) The thyroid is usually low functioning in the T21 population.

Supplement Ingredients to Avoid

Look for organic where possible. Avoid gluten, soy, dairy, corn and GMOs.

Best Defense

Your best defense is prevention. At 17 months old, Jett has never been sick. This is because he still breastfeeds and has a very good, gut-supporting diet with probiotics and a healthy variety of foods. He is not in day care, but he interacts with children from one to four times a week. I use no chemicals or antibacterial soaps in my home.

Even at four and a half years old, the only time Jett had to take antibiotics was during heart surgery when he was 6 months old. It took awhile, but his gut flora has healed since then.

Another thing I used is AIR tea, it's a Traditional Chinese Medicine concentrated herbal tea blend that cleared Jett's congestion. It takes a couple of months to fully clear everything and he took it twice a day from age 5-12 months old. It tastes great! My pediatrician recommended it. You take it before you get sick.

I used to be up a lot in the night with the nose drops and "mucus suckers" but not at all anymore. I just clean out his nose quickly with a swab in the morning and give him saline nose drops twice a day. Before, Jett was completely congested, up in his sinuses where I couldn't get to it. Now, I can feel the air coming from his nose which I couldn't do before. Jett has never had a cold or sinus infection. I give him 1/5 teaspoon (1 ml) in the morning and before bed with 2ml of water. He loves it!
