Glutathione

Oxidative stress impairs the body's ability to readily detoxify offending substances such as free radicals or to easily repair the resulting damage. It is oxidative stress that is linked to atherosclerosis, cell aging and neurologic disorders in Down syndrome.

On the other hand, glutathione is an powerful antioxidant. It helps remove free radicals and deal with oxidative stress. Studies show that levels of all forms of glutathione is low in children with Down syndrome. (And would make sense that it's low in adults as well.) This is caused by the overexpression of the SOD-1 gene.

Studies show that supplementing with B12 and folic acid can help, but you may want to look into a more direct source of glutathione.

Free radicals and oxyradicals play an important role in the development and progression of many brain disorders such as brain injury, neurodegenerative disease, schizophrenia and Down syndrome.

Glutathione is the brain's master antioxidant and plays an important protective role in the brain.

According to Dr. Jimmy Gutman, "The brain is particularly susceptible to free radical attack because it generates more oxidative by-products per gram of tissue than any other organ."

Many neurological and psychiatric disease processes are characterized by... abnormalities in glutathione metabolism and antioxidant defenses."

Generation of reactive oxygen species (free radicals) and oxidative damage are an important cause of neuron (brain cell) death from brain injury.

Chemicals that cause toxicity to certain brain cells are known to decrease cerebral glutathione (GSH), making the cells more vulnerable to reactive oxygen species (ROS). (1)

On the other hand, over-expression of the glutathione peroxidase (GPX) enzyme potently decreases cell death from brain injury. (2)

Brain Injury and Glutathione - The Gender Difference

Researchers at Children's Hospital of Pittsburgh have found that males and females respond differently to brain injury. (3)

In animal models, levels of glutathione remain constant in females who have suffered a brain injury, but drop by as much as 80 percent in males with the same injury.

When glutathione levels drop, brain cells die much more quickly. This suggests that boys with brain injuries may require different life-saving treatments than girls.

N-acetyl-cysteine (NAC), a precursor of glutathione, already approved for use by the U.S. Food and Drug Administration to treat people who have overdosed on acetaminophen, may be an effective treatment for brain injury in boys whose brains are deprived of oxygen.


Brain Disorders and Glutathione - A Genetic Cause?

Genetics researchers have found that the glutathione S-transferase gene controls the onset of Alzheimer's, Parkinson's disease and determines, not if we get these diseases, but when. (4)

The glutathione S-transferase gene has previously been linked to the risk for Parkinson's disease among people who used pesticides.

Alzheimer's Disease and Glutathione

Free radicals and oxidative damage in neurons is known to be a primary cause of degenerative diseases like Alzheimer's disease.

Amyloid- peptide (A) accumulation in senile plaques, a pathological hallmark of Alzheimer's disease (AD), has been implicated in neuronal degeneration.

Amyloid plaques encroaching on the brain increase the production of free radicals, or oxidative stress. Antioxidants, such as vitamin C and E "mop up" the damaging free radicals.

Glutathione (GSH) precursors can prevent death of brain cells induced by amyloid plaques in Alzhiemer's disease, while substances that deplete GSH increase cell death. (5)

Evidence has been piling up over the link between the amount of an amino acid called homocysteine in the blood and the chance of developing Alzheimer's.

For people not genetically predisposed to developing Alzheimer's, cholesterol and homocysteine, largely caused by an unhealthy lifestyle, are the core causal factors.

Welsh GP, Andrew McCaddon, showed that the more homocysteine that patients with Alzheimer's had, the worse their mental performance, and the worse their "cognitive impairment," the less they had of the antioxidant glutathione. (6)

Glutathione and Mood Disorders

Studies have found that the mood stabilizing drug, valproate, used to treat epilepsy and bi-polar disorder, regulates expression of the genes that make glutathione-S-transferase (GST).

In addition, chronic treatment with lithium, another commonly prescribed mood stabilizer used in treating manic-depression, also increased levels of GST.

These findings led researchers to conclude that glutathione S-transferase may be a novel target for mood stabilizing drugs. (7)

Alcohol Consumption and Glutathione

Alcohol abuse is known to impair memory and other brain functions and increase brain cell death. A new study in rats has shown that alchol consumption causes fewer new brain cells to form and results in greater cell death. (8)

But rats that were fed alcohol along with Ebselen - a glutathione peroxidase mimic that acts as a free radical scavenger - showed no similar reduction in brain-cell formation and no increase in cell death.

Substances that Boost Glutathione Levels and Protect Brain Cells

Taking glutathione itself as a supplement does not boost cellular glutathione levels, since it breaks down in the digestive tract before it reaches the cells. (Unless you take the reduced l-glutathione form, sublingually, or under the tongue. Good brands include Douglas Laboratory [the kind without NAC] or Klaire Lab. Liquid is supposed to be bioavailable as well, such as www.gshnow.com . The dosage is 75 mg/day for a child. -Andi)

However, intravenous glutathione therapy and glutathione precursors or dietary supplements are effective in boosting intracellular levels of glutathione.

Intravenous Glutathione Injections: Intravenous glutathione injections have been shown to produce amazing and rapid results, in patients with Parkinson's disease. Following even a single dosage of intravenous glutathione, many of the symptoms of Parkinson's disease rapidly improve, often in as little as 15 minutes.

Glutathione Precursors: In the Alzheimer's study conducted by Welsh GP, Andrew McCaddon, adding the glutathione precursor, N-acetyl-cysteine (NAC) to a protocol that lowered homocysteine levels by simple supplementation with B12 and folate, resulted in prompt, striking, and sustained clinical improvement in nearly all the patients. (9)  (N-acetyl-cysteine may cause leaky gut syndrome after extended use and has been seen to increase oxidative stress in those with DS, so weigh your options carefully.-Andi)

  ...a study by James et al.7 found that 800 mcg of folinic acid and 1000 mg of TMG
partially raised levels of glutathione in children with autism, and the addition of
subcutaneous injections of methyl-B12 (75 mcg/kg, 2x/week) normalized glutathione
levels.  Note that the dosage of methyl-B12 was suggested by Dr. J. Neubrander, based
on injecting it into the adipose tissue of the buttocks. (Note that Dr. Neubrander now
recommends a dosage of 64.5 mcg/kg every three days)8  The addition of vitamin B6 (a
necessary co-factor) is likely to raise levels further.  The addition of methionine may be
helpful, but it should be done with extreme caution after methyl-B12 has been given to
prevent negative reactions.8  
Vitamin C:  a study by C. Johnston9 of college students found that the addition of 500
mg/day of vitamin C raised glutathione levels 50%.  Raising the vitamin C to 1000 mg
had no additional benefit.

Cucurmin (turmeric): Studies have shown that the Indian curry spice, cucurmin, has neuroprotective effects because of its ability to induce the enzyme, hemeoxygenase-1 (HO-1), which protects neurons exposed to oxidant stress. Treatment of brain cells called astrocytes, with curcumin, increases expression of HO-1 protein as well as glutathione S-transferase. (10) (The most bioavailable brand is Longvida Curcumin.)

Ebselen: Ebselen is a glutathione peroxidase mimic and potent synthetic antioxidant that acts as a neuroprotective agent and an inhibitor of free-radical induced apoptosis (cell death). It can protect brain cells from the neuro-toxic effects of alcohol consumption. (8)

Undenatured Whey Protein: Undenatured whey protein provides glutathione precursors, has been shown to raise intracellular glutathione levels in clinical trials, and has anecdotally been reported to improve the symptoms of Parkinson's disease.

Glutathione in Brown Rice

Rice bran (tocotrienols) has glutathione peroxidase. It is also available in brown rice / whole grain rice, but not in white.

“Brown rice is high in dietary fiber, vitamins, minerals, essential oils and protective antioxidants. Why B12 & Folinic Acid for Down syndrome?

Studies suggest that an inadequate amount of vitamin B12 may be a risk factor for brain atrophy (shrinking) and possible cognitive impairment. Studies also show that individuals with Down syndrome commonly have low levels of B12 and Folic/Folinic Acid (or folate). In one study, typical seniors who took B12 supplements did a better job of holding cognitive declines at bay than placebo. Since the brain of a person with DS has similar qualities of a brain of an elderly person, we need to pay attention to studies about seniors. Supplementing with B12 and Folinic Acid also helps to normalize metabolism. 

Folic/Folinic Acid (or folate) is particularly important within our population since it is needed for the synthesis of DNA and RNA, which are the building blocks of cells. They also help prevent changes to the DNA that could lead to cancer such as leukemia, which our population is particularly susceptible to. It is also needed in both children and adults to be able to make normal red blood cells and prevent anemia. Specifically, B12 and Folinic Acid are used in a metabolic cycle called the SAM cycle.

Types of Folic Acid, Folinic Acid and Folates

Folic acid, actually, should be avoided in the DS population, those with the MTHFR variation (much more common in the DS and autism population and in mothers of children with DS) and older moms who want to have children. See   How to Prevent DS in Your Next Child in 60% of Moms for details as to why folic acid is difficult to process and properly use in our bodies. For this population, folic acid should be avoided in enriched foods such as breads and cereal as well.

Folinic acid also known as Calcium Folinate has been shown to have positive effects on children with DS whether or not they have the MTHFR variation.

Folate is a broad term, it may refer to Calcium Folinate, "naturally occuring folate" which may be folinic acid or could be a form of l-5-methylfolate. If it's not clear on the label, I'd contact the company to make sure of the source. 

Calcium Folinate has been shown to have positive effects on children with DS whether or not they have the MTHFR variation. It's not recommended for the typical population who has the MTHFR variation.

L-5-MTHFR, l-5-methyltetrahydrofolate, 5-methylfolate, etc. is the folate of choice for those with the MTHFR variation. (This is what I take. Jett has both DS and MTHFR so I give him both, just in case. Hopefully I'll eventually learn which is best but I'm covering my bases.)

B12

Different children need different forms of B12. If your child has one or more of the MTHRF mutation, the methylcobalamin is preferable (40% of the typical population has this mutation. Jett does). With some kids, if you give the full dose the first day, it causes hyperactivity. If you see this happening, reduce the dose and slowly increase each day until you reach the full dose. If problems still occur with methylcobalamin, look into the hydoxocobalamin form (see healthy awareness article below).

B12 can be difficult to metabolize so you don't want to give it in food or have them shallow it. Instead, buy sublingual drops or mouth or nose spray or other easy to assimilate form. It also needs to be taken on an empty stomach, 
first thing in the morning works well. If your child still has low levels of B12, you can look into the B12 shots (watch out for extra ingredients in the shot, however).

This is what I use for myself. I just put the drops under my tongue first thing in the morning and try not to swallow, rather allow it to dissolve in my mouth.

B12 spray
This is what I use for Jett first thing in the morning. It sprays a fine film all over his mouth so that it's more likely to be absorbed in his mouth rather than to go down his throat.

5-methyltetrahydrofolate and folinic acid as well as niacinamide which helps with the symptoms you may get from using a lot of 5-MTHF. I did get the adverse symptoms when I first took l-5-MTHF and this product would have been nice to have then.

Brain Takes Multiple Hits from Low B12 Levels

...Although her group's observational study couldn't say whether boosting B12 would prevent or reverse cognitive effects, a prior randomized trial demonstrated that high-dose B complex supplements could slow down brain atrophy in older adults.
In that study, dubbed VITACOG, supplements did a better job of holding cognitive declines at bay than placebo among those with high homocysteine levels.
"So at least from this one clinical trial it appears that [supplementation] may have some benefit," Morris argued.
Her group dug deeper into their Chicago Health and Aging Project (CHAP), measuring vitamin B12-related markers in relation to brain MRI and neuropsychological test results 4.6 years later in 121 residents of Chicago's South Side, ages 65 and older.
After adjustment for age, sex, education, race, and serum creatinine levels to control for renal function problems that could have an impact on homocysteine, all of the B12-related markers affected global cognitive scores….

..."You might be in the normal range, say in the low normal range, of vitamin B12 in your blood but still have evidence of insufficient vitamin B12 based on these biomarkers," she told MedPage Today….

Background: Environmental toxins, pathogens and host susceptibility cofactors may interact to contribute to disease. In vitro mercury exposure inhibited antiviral cytokines in human cells; however, little is known about the relationship between mercury and viruses in children. Children are susceptible to mercury toxicity; lower vitamin B-12 and folate levels and higher homocysteine levels may represent susceptibility cofactors.
This study aimed to evaluate associations between total blood mercury (Hg) and measles antibodies in children, and the influence of these susceptibility cofactors.

Design: Cross-sectional data on serum measles antibodies, Hg, homocysteine, methylmalonic acid (MMA, indicator of B-12 deficiency), and folate were obtained from the 2003--2004 NHANES for children aged 6--11 years with measles seropositivity (n=692). We used linear regression to evaluate relationships between measles antibodies and Hg, stratified by sex, MMA ?, folate b, and homocysteine?sample medians, adjusted for demographic, nutritional and environmental cofactors.

Results: Hg (range: 0.10--19.10 ?g/L) was inversely associated with measles antibodies (range: 1.00--28.24 units) in non-stratified analysis (n=692), yet positively associated among the subset of boys with higher MMA and lower folate (n=98). Among this subset with higher homocysteine levels (n=61), correlations were positive across all Hg quartiles relative to Q1 (Hg?0.20 ?g/L): Q2:?=6.60 (3.02, 10.19); Q3:?=8.49 (6.17, 10.81);
Q4 (HgN0.80 ?g/L):?=4.90 (2.12, 7.67) (ptrend=0.077).

Conclusion: Stratification by susceptibility cofactors revealed opposing directionality for correlations between Hg and measles antibodies, with positive effect estimates at lowest exposures only among boys with higher MMA, lower folate and higher homocysteine levels.

Jett's Supplement List

Happy, healthy Jett at 3 years old
Below are the supplements that Jett currently takes (it does fluctuate) to combat the effects of the 21st chromosome. Please click the links for explanations as to why, how, our experience, products etc. 

You may notice that my list is different than what is suggested in the Changing Minds Foundation website (This protocol is outdated and I don't agree with the pharmaceuticals suggested), NuTriVene's Complete Program (minus the folic acid, Piracetam, Aricept and Namenda) or LifeExtension's Treatment of DS Summary (minus the glutamine, for most kids, but Jett actually takes it). This is because the more I learn, the more I am able to pick and choose what works best for Jett.





Common Questions 
 
Where do I start?

It may be a good idea to start with a Down syndrome specific multivitamin that has been formulated by experts. All of our children have the same diagnosis, but none of our children exhibit exactly the same symptoms. So it makes sense that while many of our kids may need many similar nutrients, which a multivitamin will cover, each child's perfect blend of nutrients will be unique to him/her. So, consider starting with a multivitamin and then add supplements as needed. Some of these multivitamin companies will customize the vitamin dependant on lab results of your child as well.

How do I know what my child needs?

Each of our children is unique, so your list will eventually evolve according to your child's cues as well. There are several tools for making these decisions:
1. Get Lab Tests

2. Check for Signs of nutritional deficiency

3. Get muscle testing and or tested from a zyto machine technician

4. Compare similar children's protocols at Autism360.org

5. Keep track of your child symptoms.
You can muscle test your child to see if he can tolerate supplements that you are interested in before he even tries it. Muscle testing is available at some chiropractors, naturopaths, NAET practitioners and integrated medicine practitioners. Here's a youtube video that explains how to muscle test. This technique has worked well for Jett. 

If your child muscle tests negative for an item that you feel would benefit him, you can use NAET (Nambudripad's Allergy Elimination Technique) to "clear" the "allergen" so that your child can tolerate it. NAET has worked well for Jett. Kristen Morrison of Naturally Better Kids swears by this technique for her child with Down syndrome.

How can I support my child's intake of nutrients?

You'll want to make sure that your child is best able to metabolize these nutrients. If your child has untreated Celiac disease (more common in DS than is realized, in my opinion) or simply a sensitivity to gluten, you may want to consider avoiding gluten (found in wheat products). Gluten can irritate the fine hairs in the digestive system so much so that the hairs can fall out and cause your child's food to not be properly metabolized. So your child may be eating great foods, but isn't getting the nutrients from it because it just slides quickly by, instead of being caught in the fine hairs and slowly guided through the digestive tract. I'd hate for you to be spending a lot of money on supplements when your child can't even use them properly. Jett is gluten free to aid in digestion.

Milk can cause block your child's ability to properly process folate (in addition to other negative effects like congestion). You may want to consider avoiding milk. Jett is dairy and casein free as well. See Cerebral folate deficiency in Down syndrome for details.

Consider introducing a new thing every 7-12 days or longer (I introduce only one thing a month now) and don't introduce anything else new (food/drink) during that time.
